One-on-one Case Management
Objective:
The overall goal of our one-on-one case management is to provide needs-based support and helpful information to clients. With self-determination, people can mobilize their own resources, skills, knowledge, and capabilities so that they can maximize the use of community resources.
Approach:
- Uphold the principle of self-determination and help people make decisions by themselves
- Use a strengths-based perspective to explore one’s capacity and support systems
- Apply client/family-centered practice to identify a client/family’s needs
- Respect the dignity of clients and families
