Eligibility Criteria:
- At least one household member is 16+ & can live independently
- All household members must have legal status in Canada
- Household members do not owe money to a social housing provider & have no convictions related to housing assistance
- Household income & assets do not exceed allowable limits (amounts differ between York Region and Toronto)
Supportive Housing:
Permanent housing combined with support services for people with needs related to physical or mental health, developmental disabilities, or substance use.
Supportive Housing Program for Seniors (Toronto) Eligibility Criteria:
- Applicant is 59+ & a tenant in one of the 9 designated buildings
- In need of personal support, homemaking services, or 24-hour intermittent on-site assistance
- Medically stable & able to direct your own care
The Access Point Supportive Housing (Toronto) Eligibility Criteria:
- Applicant is 16+, lives in Toronto, & qualifies for a housing subsidy
- Have mental health and/or addiction concerns
Streamlined Access Programs (York Region) Eligibility Criteria:
- Applicant is 16+ & lives in York Region or South Simcoe
- Dealing with a serious mental health concern, addiction, and/or a dual diagnosis of mental illness & developmental disability
Housing with Supports (York Region) Eligibility Criteria:
- Applicant is 18+, requires supervision in daily activities, is in need of financial help, & not eligible for long term care
- Applicant must either: Receive mental health services or have a mental health diagnosis, have a disability, or be a frail, elderly person
Emergency & Transitional Housing:
Emergency housing is short-term accommodation for people who are homeless or in crisis, where you can get basic necessities.
Transitional housing is supportive, temporary accommodation that offers structure, supervision, & life skills, bridging the gap from homelessness & shelters to permanent housing.
